We invite all participants to do some thinking prior to the start of the Leadership Series by completing an application and preparing a family diagram of three generations. The application is a series of questions to prepare you for the perspective and process of the program. Resources for completing the family diagram are available on our website.

For "new" applicants, a conversation with one of the group consultation facilitators will follow the receipt of your application.

Family Information

The initial steps toward understanding self and family entail a beginning grasp of one’s own immediate family, family of origin, and extended family. Please write brief overviews of the following:

1. Your immediate family/relationship system (nuclear family, current household).

2. Your family of origin (the family in which you were raised), including the facts that you presently know about them.
